Pneumococcal protein-binding-protein 4 (PBP4) is an important determinant of virulence and host immune evasion of Streptococcus pneumoniae. This study tested the hypothesis that PBP4 promoter polymorphism influences susceptibility to invasive pneumococcal disease (IPD) and carriage in children. PBP4 promoter sequence polymorphisms were analysed in 164 children with IPD and 131 healthy controls. A cohort of children with pneumococcal carriage was followed prospectively over a period of 19 weeks to determine PBP4 promoter polymorphisms and to measure PBP4 mRNA levels. PBP4 promoter polymorphism was associated with increased risk of IPD (odds ratio 1.3, 95% CI 1.2-1.5) and carriage (odds ratio 1.2, 95% CI 1.1-1.4). In children with carriage, a significant correlation was found between the levels of PBP4 mRNA and the number of polymorphic repeats in the PBP4 promoter (P=0.01). No significant difference in expression level was found between the PBP4 promoter polymorphisms in children with IPD.
